UrPlan is Includa’s planning approach for NDIS participants who want clarity, direction, and a practical way to think about the future. 

Rather than beginning with funding categories or services, UrPlan starts with what matters most: who you are, the people in your life, and what a good life looks like for you. 

Includa works alongside you and the people in your life to create planning conversations that are practical, visual, and focused on action. 

Planning that leads to action

UrPlan brings together the people who know you best to build a shared picture of the future and identify practical next steps. 

The process helps you: 

  • explore what is important to you and what gives your life meaning
  • recognise strengths, relationships, and community connections
  • imagine realistic possibilities for the future
  • turn ideas into clear, achievable goals
  • connect everyday actions with long-term direction

UrPlan draws on established person-centred and PATH planning approaches, focusing on strengths and opportunities rather than problems or deficits. The plan grows and adapts as your life changes. 

UrPlan is not an NDIS plan review or service meeting. 

It’s not about line items, budgets, or compliance paperwork. 
And it’s not a document that sits unused after it is written. 

UrPlan is a living plan designed to support learning, adjustment, and forward movement over time. 

How the process works

Getting ready

Before the planning session, we spend time getting to know you so the process feels comfortable and meaningful. Together we explore: 

  • what matters to you right now
  • who you would like involved
  • where and how the session will take place
  • your strengths, interests, and hopes for the future 

Two trained facilitators guide structured conversations using visual planning tools to help you: 

  • reflect on where you are now
  • imagine where you would like to be
  • identify meaningful goals
  • map practical next steps 

What happens next

After the session, Includa supports you to turn ideas into action. UrPlan evolves as life changes, helping you stay connected to what matters most. 

Who UrPlan is for

UrPlan requires a commitment of around 25 hours of funding over approximately 12 weeks to support preparation, planning, and follow-up.
